Waiver

All climbers must have a signed waiver on file before using the gym. Climbers under the age of 18 must have the waiver signed by a parent or legal guardian. Under normal circumstances, a parent should accompany the minor to the gym for the first time to fill out/drop off the waiver. Once the waiver is on file, the climber may return with a different chaperone or, if over 12, on his/her own.

We understand that in some cases, such as birthday parties and out-of-town visitors, it is not possible or practical for parents to accompany their children to the gym. In these cases, parents may sign the waiver in the presence of the adult who will be accompanying the minor, or a notary public. The accompanying adult or notary should then sign the waiver in the place designated for a witness.

Download the waiver and print it from your computer. To view and print the waiver you need to have the Adobe Acrobat reader installed and configured. If you don't already have it, the free Acrobat reader can be downloaded from Adobe.

Alternately, you can email us at info@portlandrockgym.com and ask us to mail or fax a waiver to you.

Filling out the Waiver

Adults

Climbers 18 years of age or older may fill out a waiver available at the front desk upon arrival at the gym.

Parents of Minors

If you will not be accompanying your child to the gym, you must fill out the waiver in the presence of the adult who will be accompanying the child, or a notary public.

Print your name, and then the climber's name in the appropriate places at the top of the page. Read and initial each paragraph.

Leave the belay experience paragraph blank unless filling out a waiver for a youth over 11 who has belay experience. Everyone must pass our belay test before top-rope belaying at the Portland Rock Gym. To lead climb or lead belay, you must also pass our lead test.

Remember to sign and date on the parent's signature line (parent or legal guardian).

Chaperones should check over the waiver to be sure it is filled out correctly before signing as a witness on the "other third party" signature line at the bottom. Party or group chaperones should witness all parent signatures prior to the event, then sign the "witness" signature line upon arrival in the presence of PRG staff. If you are accompanying your child to the gym, you can leave this line blank. Fill in the climber's full name and address information in the boxes at the bottom.

Children's Policy

Children under 10 years old are not permitted in the bouldering area. This age must be accompanied at all times by an adult that is not climbing or belaying.

Children under 14 must be actively supervised by a trained/experienced adult when in the bouldering area. Exceptions may be made for children with an Annual Memberships and significant climbing experience (determined by PRG staff).

Children under 14 are not allowed in the gym without a chaperone. Climbers must be at least 12 to top-rope belay. Climbers must be at least 14 to lead belay.
